Importantly, predicted doses on this website should not be interpreted as dosing recommendations for clinical use. For endorsed dosing guidance, refer to the Dutch Pediatric Formulary (www.kinderformularium.nl, or its international affiliates) and the Dutch Teratology Information Service, Netherlands Pharmacovigilance Centre Lareb (www.lareb.nl). These institutions base their recommendations on a thorough benefit-risk assessment of available evidence, sometimes incorporating pharmacokinetic models. Formularies strive to issue guidance integrating recent evidence, but delays may occur. Hence, dosing recommendations from these sources may not (yet) integrate simulation results.
